• Hanson High School senior planning to pursue a post-secondary education
• List high school activities (student government, sports, publications, school sponsored community service programs, student committees, arts, music, etc.), including dates/year of participation and office held or leadership role.
• List community service and/or civic activities (ie. 4-H, scouts, religious organizations, etc), including activity, role, dates and average hours participated.
• List awards, honors, publications or special recognitions that you have received, including year.
• Describe in one detailed paragraph the most significant personal challenge you have had and how you overcame that obstacle.
• Have 3 individuals complete the attached Character Reference sheet.  Individuals may include your high school counselor or advisor, an instructor, work supervisor or anyone who knows you well, but is not related.

Amount $1,000 (non-health related major) $1,500 (health related major)
