Eligibility
Requirements
  • Must be between ages 16-25
  • Must have a HS diploma, GED or a HS student in good standing who will graduate in current academic year
  • Dependent of a parent who suffered either an on the job fatality or injuries that have had significant adverse effect on family income
  • Proceeds must be used for tuition, fees, room and board and will be paid directly to institution
  • Accepted into a post secondary institution

Required Materials

  • Completed application (attached)
  • TWO letters of recommendation from someone other than a relative
  • Narrative (2pg max) describing parent's work related accident, its impact on you and the family and why  the scholarship will help you attain your goals
  • Copy of First Report of Injury
  • Copy of completed FAFSA

Amount: Contingent on need and availability of funds
